Ways to facilitate a pleasant visit:
Upon Check in, please have the following ready: (Current Insurance cards, Photo ID, Co-Payment, Referral for visit). Bring all medication bottle to every clinic visit. If you require copies of your labs, please ask the Medical Assistant at the start of your visit. Report any recent hospitalizations. Please request prescription refills during your visit. Have your questions ready…make a list. ake the lab order you receive at your visit with you to your testing facility. Please call us prior to any medication changes from you other physicians. Notify us if you change primary physicians. Have your labs and or any testing completed prior to your visit.
In order to ensure that your visit is as swift as possible, please feel free to print and complete any of the following forms applicable to your visit. Arriving with all necessary paperwork greatly assists our staff in creating and updating your file while limiting your overall wait time.

Pay Your Bill
If you would like to contact our Billing service please phone (219) 738-6726 or you may pay your bill online at the link below. If you wish to pay online, you will need the invoice number and the patient’s last name.
We encourage our patients to utilize our Patient Portal via Health Companion. The patient portal allows you access to your health information including your health history, clinical summaries, lab results, medication list and more.
