About Dr. Saleh

Dr. Mohammad Saleh is a community nephrologist working in Illinois and Northwest Indiana with extensive experience in hospital-based medicine, outpatient clinical care, and telemedicine.

He focuses on bridging the gap between inpatient and outpatient care — optimizing patient education and insight to slow chronic disease progression and reduce hospitalizations. He is board-certified in Internal Medicine and Nephrology, with additional board certification in Pediatrics.

Dr. Saleh graduated from the University of Illinois at Chicago and completed his residency in Internal Medicine and Pediatrics at the Medical College of Wisconsin. He completed his fellowship in Nephrology and Hypertension at the Medical College of Wisconsin, adding an additional year of basic science research focusing on hypertension.

In addition to his clinical practice, Dr. Saleh serves as Medical Director of several dialysis units and is a member of quality review committees at various hospitals. He is also an advisor to a technology company building a mobile platform to help patients communicate more effectively with their care teams.
