Dr. Mohammad Saleh, M.D
Dr. Mohammad Saleh MD is a community nephrologist working in Illinois and Northwest Indiana. He has extensive experience in hospital-based medicine, outpatient clinical care and telemedicine. He focuses on bridging the gaps between inpatient and outpatient care with the goal of optimizing education and patient insight to slow down chronic disease states and reduce hospitalizations. He is board-certified in Internal Medicine and Nephrology. He graduated medical school from the University of Illinois at Chicago and went on to complete his residency in Internal Medicine and Pediatrics at the Medical College of Wisconsin in Milwaukee. He completed his fellowship in Nephrology and Hypertension at the Medical College of Wisconsin while also participating in basic science research focusing on hypertension, adding an additional year of training.
In addition to his clinical practice, he is also an advisor to a technology company that is building a mobile and web-based platform. This technology helps patients communicate more effectively with their care teams and providers. In addition, he is the medical director of several dialysis units and is a member of quality review committees at various hospitals. His hobbies include running, competing in triathlons and coaching youth soccer. He lives with his wife and 4 children in the Chicago-land area.
